I went to the Hospital for a Colonoscopy Yesterday (went smoothly) but during the Pre Operation Questioning I realized that they Had AN INCORRECT RECORD of my Health History... They have me diagnosed with BI POLAR Disorder but That is NOT TRUE. I Had CANNABIS PSYCHOSIS... a Totally Different Mental Health Challenge... The Doctor at the Mental Hospital Wrote Down the WRONG Diagnosis because he was Uneducated about Medicine... I TOLD HIM THE CORRECT DIAGNOSIS but he Didn't Understand... So... He Prescribed LITHIUM... The Totally Wrong Prescription... That made my Mental Health WORSE... DUH! The Correct Action is to Simply Stop Smoking Marijuana and In a Month The Effects Just Go Away... So anyway... I told the Nurse that the Record was wrong... but she was Unable to Actually Change it... SO That Incorrect Information Remains in My Record!
Corrections:

If the provider or plan does not agree to your request, you have the right to submit a statement of disagreement that the provider or plan must add to your record.
